• Original and copy of certificate 086-u. If the document is not in Russian, it is mandatory to attach a notarized translation of this document into Russian.
• Original and copy of the fluorography results (if not specified in the 086-u certificate). The fluorography results are valid for one year, but no less than 2 months before the expiration date. If the document is not in Russian, it is mandatory to attach a notarized translation of this document into Russian.
• Original and copy of the Vaccination Certificate or extract from the vaccination card. If the document is not in Russian, it is mandatory to attach a notarized translation of this document into Russian.
• HIV and syphilis test results (valid for 2 weeks). If the document is not in Russian, it is mandatory to attach a notarized translation of this document into Russian.
• Copy of medical insurance policy.
