- Медицинская справка № 086-у
- Цветное фото (размер 3х4)
- Копия лабораторных исследований крови на сифилис (RW-отриц. или РМП/РПР- отриц.)
- Копия лабораторных исследований на ВИЧ (анализ крови на ВИЧ)
- Нотариально заверенное согласие законных представителей на заселение в общежитие несовершеннолетних граждан (в случае отсутствия законных представителей при заселении)
